Below are the referral steps and follow up process:

1. Complete and submit the referral form below. You will be asked to choose one of our three core dignity programs.
2. Receive an email confirmation with:
   • Acknowledgement of your referral
   • Details of the one-on-one shopping date with your referred participant and a Dignity Foundation representative
3. After the shopping trip, receive a follow-up email with:
   • Details of the visit
   • A sample selection of items purchased
